Vivent Health's social work case managers have expert knowledge of the health care system to help patients get enrolled in quality HIV health care and to make sure that their health care benefits are secured. In addition to helping secure access to health care, the team will help patients receive necessary social services that help ensure their best chance for treatment success.

This program provides:

• Comprehensive needs assessment
• Creation of a care plan
• Enrollment in medical care
• Advocacy for your benefits
• Enrollment in other health and social services
• Transportation assistance
• Housing and financial assistance
• Support Groups

Vivent Health Social Services are available to people living with HIV. If you are currently a Vivent Health patient or client and would like to make an appointment, or if you have HIV and would like become a Vivent Health social services client, please fill out the appointment form on the website or call 800-359-9272.

When you enroll in Vivent Health case management system a social work case manager will become your partner in planning the best way for all of your needs to be met. Your case manager will work together with you, your health care providers and others involved in your care so that you have a complete approach to meeting your needs.
