Family Health Centers-Phoenix provides primary care services for children. Primary care includes:

-Well-child-Well-child checkups are due at age 2-4 weeks, 2, 4, 6, 9, 12, 15, and 18 months, 2, 2½, and 3 years, and yearly after age 3
-Sick visits-When your child is sick and you need to see your provider soon, call your Family Health Center site and ask to speak to a triage nurse
-Immunizations-Immunizations are provided at each health center, including those required for preschool and primary school entry

Family Health Centers welcomes new patients. In order to give the best care to our current patients, not all Family Health Centers sites are able to accept new patients all of the time. Be sure to call to find out if we are accepting new patients.

Family Health Centers accepts most private insurances, Medicare and Medicaid. If you have Medicare you do not pay a deductible when you visit Family Health Centers. If you are uninsured, Family Health Centers can help you sign up for free or low cost health insurance. We have staff on-site to help complete applications through kynect, Kentucky’s Healthcare Connection.

Family Health Centers, Inc. provides services without regard for a person’s ability to pay. We use a sliding-fee-scale to determine financial assistance “discounts” based on the family or individual’s income and family size. Sliding-fee discounts are based on the Federal Poverty Guidelines. If you or your family uses the sliding-fee-scale, then you are assigned to a Pay Class (Pay Class A, B, C, D, E, or F).

-Pay Class A patients pay a nominal fee up-front at each visit. The nominal fee is $20. The Pay Class A nominal fee covers your visit, labs, and x-rays. Shots, flu shots, supplies, and prescriptions are not covered under the nominal fee, and you may be billed.
-Pay Class B – F patients pay a minimum fee up-front at each visit. The minimum fee varies by Pay Class. The minimum fee covers part of your total visit charge. You will be billed for the rest – minus your discount. The total charge for your visit includes the visit to the provider, and other services you receive such as lab tests, x-rays, immunizations or family planning services.

Proof of income is required to determine your Sliding Fee Pay Class. You must bring proof of all family income to your first appointment. Once a year, FHC will ask you to “re-certify” your sliding-fee-scale “discount”.

We are accessible for those with hearing, vision, or mobility limitations. We provide interpreters for any patient who cannot communicate in English. We also provide a number of translated forms and handouts in other languages. Let a staff member know if you need an interpreter or help reading a form.
